ACTIVITY DIRECTOR
Summary
Plans, organizes, and directs approved recreation program for residents in nursing home by performing the following duties.
Essential Duties and Responsibilities
Include the following. Other duties may be assigned.
· Directs and organizes therapeutic activities to assist residents to meet care plan objectives.
· Responsible for ensuring seven (7) day activity plan is developed and adhered to.
· Regulates content of program in accordance with residents’ abilities, needs and interests. Program must meet Federal and State guidelines.
· Maintains documentation on each resident including assessments, care plans, and progress notes. Documentation reflects residents’ reactions and evidence of progress or other change of condition.
· Counsels and encourages residents to develop independent leisure activities.
· Organizes and coordinates special outings and accompanies residents on outings such as ball games, sightseeing, and/or picnics.
· Develops monthly activities that are seasonal and appropriate for the type of resident served.
· Maintains accurate financial records for the department.
· Prepares and submits requisitions for needed supplies.
· Understands and adheres to the guidelines of “Residents Rights”.
Certificates, Licenses, Registrations
Certified Therapeutic Recreation Specialist or eligible-awaiting certification. Certified by NCCAP or the State of Texas as a Certified Activity Director or eligible for certification.
Physical Demands and Work Environment
Employee is required to hear, speak, sit, stand, and walk daily. The position is located in healthcare facility environment. Employee may lift up to 40 pounds and/or transfer 100 plus pounds.
Must be in good general health and demonstrate emotional stability in order to cope with the mental and emotional stress of the position. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
ACTIVITY ASSISTANT
Reports To: Activity Director
Summary
The Activity Assistant aides the Activity Director in planning, organizing, and implementing programs of therapeutic activities. These activities are designed to meet the social, spiritual, intellectual, emotional, and physical needs and interests of residents in accordance with the comprehensive resident care plan. Assistants work closely with residents, family members, volunteers, staff, and community representatives.
Essential Duties and Responsibilities
Includes the following. Other duties may be assigned.
· Acts as the leader for scheduled activities such as exercise, music activities, socials and snack gatherings, etc.
· Interacts and facilitates activities with residents (one on one) usually in-room.
· Keeps daily attendance records of residents’ daily participation, as well as daily documentation of 1:1 visits.
· Assists with the transporting of residents to and from activities and other times as needed and/or requested.
· Accompanies residents on field trips outside the facility.
· Assists Activity Director in meeting goals for residents and department.
· Participates in Residents Care Plan.
· Understands and adheres to the guidelines of “Residents Rights”.
· Participates and attends periodic in-services and staff meetings.
· Assists other department heads when necessary.
Qualifications
Previous experience in leading recreational activities, arts/crafts, intellectual and educational activities, community outings, etc., desired.
Education and/or Experience
High School Diploma with course work in recreation is desirable. Ability to read and write required.
